A consistent intake and case-management flow is what turns single-service contacts into measurable family outcomes — and produces reportable CSBG data as a by-product.

The flow

  • Screening: quick eligibility and urgency check, same day where possible
  • Intake: verified documents, household composition and consent
  • Assessment: strengths, barriers and household goals
  • Service plan: services, referrals, owner and review date
  • Follow-up: scheduled contact points, not ad hoc
  • Exit and outcome: record the outcome achieved and the evidence for it

Bundle services deliberately

Households arriving for a utility crisis are often eligible for weatherization, food support, Head Start or workforce services. Build the cross-referral prompt into the intake form so the offer is made every time instead of depending on who is at the desk.

Data quality by design

Capture demographics once, at intake, and reuse them. Map every service and outcome field to the national performance indicator it feeds, so annual reporting is an export rather than a reconstruction.
